The first step towards registration with Making Waves is completion of the enrolment form below. After submitting this form, you will be contacted by one of our executive members to ensure you are still interested in having your child participate. The registration fee consists of a donation (amount suggested is $30) and is due on your child's first day of lessons, payable at the pool by either cash or cheque.

Please note: children already participating in the Fall 2012 lessons do not need to re-register for the Winter 2013 lessons on this form.
